These couplets are there in the collection "Tarkash" by Javed Akhtar and my love for them has been growing as I have been growing up.
As far as I remember there is another couplet to add to the ones you have already mentioned. (Though you are not in the wrong factually cuz these are the ones that Jagjit Singh rendered in the album Silsila). If I remember it right it went something like:
Ek ye ghar jis me mera saaz-o-saaman rehta hai;
Ek wo ghar jis me meri boodhi Naani rehti thi.
